Minimum Skills/Requirements

Two to three years college level courses/Associates Degree/Diploma in Nursing. BSN preferred. State of New Mexico R.N. license required. NM Drivers License required. Less than one year experience in nursing.
Requires detailed knowledge of nursing practice and care delivery in terms of what and how work is to be done as well as why it is done. The individual must demonstrate knowledge of the principles of growth and development over the life span of patients served.
Primary Job Functions

Responsible for the primary nursing care and coordination of care for assigned homecare /hospice patients of all ages. Develops a plan of care appropriate for the patients needs in consultation with the physician. Supervises the home health/hospice aides.
. Possesses excellent patient assessment skills. Uses the nursing process and nursing diagnosis to create a plan of care for each assigned patient. Identifies patient problems by use of patient assessment, intuition, decision making, use of other resources (written and consultative), listening and communication skills.

. Directs and coordinates care with patient, family, all other disciplines, primary and secondary physicians and community resources/programs.

. Assesses and directs patient and family teaching needs and is proficient in the use of return demonstration and teaching aides in the home setting.

. Ensures that patients understand their rights and responsibilities, advanced directives and the meaning of informed consent during the admission process.

. Maintains a proficiency in the use of all equipment used in his/her practice, such as O2 oximeter, O2 concentrators, scales, doppler, rehabilitation aids, infection control, IV and Central lines, feeding, apnea monitors, and nebulizers.

. Organizes his/her day and schedule around infection control issues, cost:effective issues, and patient requests.

. Performs on:call duties effectively with the ability to triage patients over the phone and visit when needed.

. Directs own educational needs for the top 10 ICD9 codes and/or diagnoses for SGH HHC: CHF, COPD, diabetes, wounds/post:surgical, arthritis, newborns, Coumadin dependency, malnutrition/ nutrition needs, medication instruction, pain and symptom control/hospice care.

. Supervises the home health aides in compliance with the Medicare/Medicaid rules and regulations. Assists with orientation of staff and performance improvement indicators.

. Performs other functions as required.
